The RN Case Manager uses the nursing process to develop, coordinate, and manage the plan of care for hospice home patients as a member of the Interdisciplinary Team (IDT). RN Case Manager is responsible for admitting and coordinating appropriate care for each patient and family to include and not limited to admissions, assessments, discharges and PRN visits, On Call Rotation. Qualifications Licensed RN in the State of Texas - required 1 years' experience as a RN - Preferred experience with Hospice or Home Health - Effective written and verbal communication.

Pay: $34.00 - $38.00 per hour
